The present report is a case study of a male patient with anuric end-stage renal failure who developed metabolic encephalopathy associated with cefepime use. This case study suggests that the role of cefepime in producing methabolic encephalopathy may be underestimated. Thus, administering cefepime to severely ill patients, or those with impaired renal function should be done very judiciously, recognizing the encephalopathic potential of cefepime.
