Abstract
The definitive care of a missile wound crossing the midline presents challenging problems, requiring a carefully planned, aggressive surgical approach. Extent of missile track, integrity of ventricular system and basilar dura, and retention of metallic foreign bodies are the principal factors which the surgeon should take into account if grave complications are to be prevented. Illustrative cases with long-term follow-up evaluation are presented.
