To the Editor: Atrial infarction is an underrecognized clinical entity, and atrial rupture is a complication in fewer than 5 percent of the cases diagnosed.1,2 We report a rare case of an isolated right atrial infarction with rupture and epicardial hematoma formation that presented as a cardiac mass.3 An 80-year-old man with chronic renal failure treated by dialysis presented with dyspnea but no pulsus paradoxus, Kussmaul's sign, or evidence of left ventricular failure. The chest radiograph revealed an enlarged cardiac silhouette. Uremic pericardial effusion was suspected, but two-dimensional echocardiography revealed a mass distorting the right atrium and right ventricle. This.
